To facilitate your completion of the online screening form, depending on the scope of the background checks being undertaken, you will require some or all of the following information:

• Your personal details, including your full name, all other names known by, date of birth, ID number(s).
• Identity documents, including your passport(s) and other identification documents;
• Full address details of where you have lived for the past five years, both in your home country and overseas (please account for the full five years, leaving no gaps);
• Full details of all your employment history (i.e. dates of employment, positions held and company contact details, etc.);
• Full details of all educational establishments you have attended;
• Full details of any professional organisations or technical qualifications which you hold;
• The name and contact details of personal referees who will be able to confirm gaps in employment. These people should be over 18 years old, not related and not living at your current address.

If your screening includes a basic criminal record check from the Disclosure and Barring Service ("DBS"), both a paper and online certificate can be produced by the DBS and sent to a recipient of your choice.

The DBS have published a Privacy Policy for applicants of basic DBS criminal records checks and a link to their website where you can review their policy can be found here.

In addition to completing the form, your original ID documents must be verified by the HR Department of Hippo Digital, please supply your original ID documents to HR as soon as possible. For a list of acceptable documents, please refer to the DBS website. Click here.

Once you have completed all of the tabs within the screening form, you must click 'Submit'.  

At the end of the application you will be asked to electronically sign a consent form. This consent form grants permission to CREDENCE BACKGROUND SCREENING and its agents to carry out the enquiries necessary to complete your background checks.  Once submitted, you will receive by email a copy of the consent form.
